Less than 50 lbs: $50
More than 50 lbs: $100Pets larger than 65 lbs are not eligible for group cremation due to the size of our equipment. Private Cremation prices apply to pets over 65 lbs.

Group Cremation ashes can be returned to the family; (acknowledgement and signed consent form required); you will be getting your pet’s ashes along with many others back, mixed together.
Group cremation ashes take longer to get back than individual cremation.
Group ashes may also be scattered at owner’s request.

Authorization: I certify that I am the owner, or authorized agent of the owner, of the pet described above and that I am 18 years of age or older. I understand that cremation is an irreversible process, and I hereby authorize cremation
I understand that this process is irreversible.
Unclaimed Remains: I understand if my pet's cremated remains are unclaimed for 60 days after notification of cremated remains availability, the Crematory reserves the right to dispose of the cremated remains in a dignified manner.
Items such as blankets, beds, toys, collars, etc. that are with your pet when brought into the care of Levi’s Legacy Pet Cremation will be disposed of unless specifically requested to be returned.
Release and Certification: I agree to release and indemnify the crematory, agents and employees, from any claim, liability, cost or expense resulting from their reliance on or performance consistent with the directions, authorizations and agreements herein. I agree that the Crematory’s liability for negligent acts (of itself, agents, or employees) is limited to a refund of the cremation fees paid for this pet.
I warrant that all representations and statements contained in this form are true and correct. I have read, understand, and agree with this document.
I understand that cremated remains should be available within 5-7 business days after drop-off/pickup, unless otherwise scheduled with the crematory staff. (Group cremations may take longer)

We all have that one heart animal that shapes who we are, how we love, and the entire construct of what “family” means.
Levi was that for our family. We were lucky to have twelve years with our friend. He unfortunately passed in an untimely way, leaving our family shocked and beyond broken. We still feel the hole in our hearts where he should be.
We want to honor his legacy and the love he gave our family by helping other families go through the loss of their heart animals.
